Job Description: Case Worker
Location: Gqeberha, South Africa

Key Responsibilities
1. FOTD Intake and Contracting
Intake of pregnant clients on first tri mester of pregnancy into FOTD program based on stipulated Ubuntu Intake procedure and processes with 100% enrollment by end of financial year.
2. Home Assessment and Communication
Assess FOTDs client's home situation after delivery within 7 days through home assessment.Ensure FOTD homes are safe and secured through quarterly reassessments.
3. Mental Health Assessment and Care Planning
Assess the mental health of your FOTD intake clients and others in your caseload using the mental health screening and establish the severity of the situation.
4. Individual and Group Counselling and Impact Evaluation
Plan, organize, and lead structured and impactful individual and group work.
Monthly group evaluations and impact reports during end of program.
5. File Management and Data Confidentiality
Maintain file updates, submit tick registers, workplan and ensure data confidentiality.
6. Other Duties
Facilitate application of birth certificate to all FOTD babies
Prepare monthly quality report.
Collaborate with other stakeholders and professionals to meet clients' needs.
Use IMS criteria for caseload management and ensure clients are consulted based on the IMS criteria.
Requirements
Bachelor of Social Work (Registered with SACSSP) or BPsych Degree (Registered with HPCSA)
5 years experience in an NGO/NPO sector
Background in FOTD programme
Skills and Knowledge in addressing psychosocial issues related to pregnancy, including potential risks and concerns.
HIV Management Care
Knowledge of the Family Act
Knowledge of adherence counselling
Knowledge of mental health related issues
Previous knowledge or experience of working with individuals, families and groups.
Fluent in IsiXhosa and English (inclusive of reading and writing)
Good communication skills
Location: Port Elizabeth
Start date: 1 August 2025.
